If the peripheral blood counts are routinely followed in patients receiving the drug, it is most likely that serious toxicity can be completely avoided. The most frequent side effects involve mild drowsiness, diplopia, and gastric irritation. These are usually dose related and occur in about 5% of the population. The relatively low incidence of toxicity makes carbamazepine an extremely valuable anticonvulsant drug, and this should be carefully noted in relation to the development of new anticonvulsant drugs.

Only a small number of patients experienced temporary improvement, but no data were presented. In the same manner, Peterman (1954), without presenting data, states that the compound was effective in only 2 of 68 patients. Kaplan and Maslanka (1952) briefly reported greater than 50% reduction, principally for generalized tonic-clonic seizures, in 16 of 31 patients. In contrast to the previous studies, Hawkes (1952) obtained beneficial results in 39 of 59 patients observed for two years. The drug was used alone in all but 11 patients.

Usual doses are in the range of 2-3 mg/kg and rarely exceed 5 mg/kg. Doses in excess of 250 mg/day are almost always associated with toxicity in the form of sedation, lethargy, and mental dullness, which is the principal toxic effect of the drug. The other major toxic reaction is restless, irritable, hyperactive behavior with short attention span, especially in children. Nevertheless, the ubiquitous clinical use of phénobarbital attests to its relative safety with chronic administration. The long plasma half-life of phénobarbital in man, 3-6 days, results in extremely stable plasma concentrations and permits once daily dosing, which is a distinct advantage.
